AZ Foundation Special Enrollment

Arizona Foundation for Medical Care (AFMC)’s contract with the Mayo Clinic will terminate effective March 31, 2007.  State of Arizona employees and retirees enrolled in AFMC are being offered the opportunity to make changes in their medical coverage through a special enrollment period during February 2007.

A Special Enrollment packet was mailed to AFMC members.  If you do not receive your packet by February 15, 2007, please contact AZ Benefit Options at 602-542-5008 or 1-800-304-3687.
