Protect Yourself and Your Family – Get Vaccinated

IMPORTANT NOTICE: Benefit Options Wellness has begun planning for the 2011 Flu Shot Program to operate October through December. Schedules to come.

Schedules:

State of Arizona employees and Benefit Options members (retirees, spouses and dependents 4 and older) have three options to receive a FREE flu shot through the Benefit Options Flu Shot Program

In addition to the options listed above, ASU and UA faculty and staff click their respective link to access the on-campus flu clinics offered by the university wellness programs.

WARNING

You should not receive vaccine if you:
1. Have ever had a serious allergic reaction to eggs or to a vaccine
2. Have a fever, acute respiratory infections, or other infections or illness
3. Have a history of Guillin-Barre Syndrome
