Mini Health Screening

Benefit Options Wellness contracts with Healthwaves to provide the Mini Health Screening. The components of the Mini Health Screening are consistent, but the method of screening is different.

Blood pressure
Body composition (weight and body fat percentage)
Fasting cholesterol and full lipid panel*
Fasting blood sugar*

* With Healthwaves an 8-hour fast is recommended for best results.

Optional Tests:

• Osteoporosis: Free for women 40 and older. $35 copay for women under age 40.
• PSA (prostate-specific antigen) $5 for men 40 and older. $40 cost for men under age 40. (Offered by Healthwaves only)

Screening Results:

• Healthwaves: The fasting screening requires a blood draw which must be evaluated in a lab. Individual, confidential results will be mailed to each participant. A copy of the test results can also be sent to the participant’s physician (participants need to provide physician contact information at the event if they want the report sent to their physician)

Cost:

• Main screening components are FREE
• Optional tests costs are listed

Length:

• Each screening takes approximately 10 to 15 minutes
• Total event time will be based on the size of the work site

Participation:

• A minimum of 25 participants is required
• Exception may be considered for the rural areas on a situational basis

Osteoporosis Screening

Benefit Options Wellness contracts with Healthwaves to provide a bone density screening using a heel DEXA scan machine. The Osteoporosis screening may be requested as a stand alone service or combined with the Mini Health Screening.

Screening Results:

• Screening results will be immediate, and follow up recommendations will be made on the findings

Cost:

• FREE for women 40 and older
• $35.00 copay for women under age 40

Length:

• Each screening takes approximately 7 minutes

Participation:

• A minimum of 20 participants is required
• Screening events are scheduled for a minimum of 2 hours

Mammography Screening

Benefit Options Wellness contracts Mobile Onsite Mammography (MOM) to provide Mammography (Breast Cancer) screening. The MOM mobile travels to perform mammography screening at work sites across Arizona.

Screening Results:

• Confidential mammogram reports (results) will be sent to the participant and the physician they designate
• Patients must provide the contact information if they want the report sent to their physician

Cost:

• MOM will directly bill the Benefit Options insurance*.
• EPO and HSA Members do not pay a copay at this screening it is covered 100%
• PPO members must meet the $500 deductible before this screening is covered at 100%
• * Participants will need to show their Benefit Options insurance card at the time of their appointments

Length:

• Each screening takes approximately 10 to 15 minutes

Participation:

• A minimum of 20 participants is preferred
• Appointments are required to participate
• Non-Benefit Options member may participate, but other insurance copays may apply

Skin Screening

Benefit Options Wellness contracts with Healthwaves to provide a facial skin screening using a Visiderm machine. The skin screening may be requested as a stand alone service or combined with the Mini Health Screening. The screening uses a black light to show areas of sun overexposure and potential damage on the face.  It is not a diagnostic test, but is an analysis.

Screening Results:

• Screening results will be immediate, and follow up recommendations will be made on the findings

Cost:

• FREE for benefits eligible employees and Benefit Options members

Length:

• Each screening takes approximately 5 minutes

Participation:

• A minimum of 20 participants is required
• Screening events are scheduled for a minimum of 2 hours

Prostate Cancer Screening

Benefit Options Wellness contracts with Prostate Onsite Projects (POP) to provide Prostate cancer screening. The POP mobile unit travels to perform prostate exams at work sites across Arizona.  The screening includes; prostate specific antigen (PSA) blood test, digital rectal exam (DRE), testicular exam and a physician consultation.s Arizona.

Screening Results:

• Results are immediate and will be discussed with patients by the physician.  The PSA blood test results will take 7-10 days, and patients will receive a call about their results.

Cost:

• FREE for Benefit Options health plan members (including spouses) and benefits eligible employees
• POP will directly bill the Benefit Options insurance for UnitedHealthcare and BCBS/AmeriBen* and ADOA will pay for the cost for Aetna and CIGNA members and eligible employees not enrolled in the medical plan
• This screening does not have a copay

* Participants will need to show their Benefit Options insurance card at the time of their appointments

• Appointments are required to participate
• Screening events are schedule for a minimum of 6 hours.
