2008 FLU VACCINE PROGRAM

STATUS: The 2008 Benefit Options Wellness Flu Vaccine Program is in its final phase and will run through December 31st.

SCHEDULES
The worksite flu clinics are nearly all completed, but during the final phase of the Flu Vaccine Program employees and Benefit Options members can receive FREE flu shots at any of the Healthwaves public clinics. To view the Healthwaves schedule and find a clinic near you click the link below.

ALTERNATIVES

If you are located in a rural area and there are no Statewide or Public events scheduled near you, you may call Healthwaves to find an alternative. Call Healthwaves at 480-968-1886 and please identify yourself as a State of Arizona employee/Benefit Options member. Healthwaves has agreements with some other organizations or may be able to accommodate reasonable requests.

WARNING
You should not receive vaccine if you:
1. Have ever had a serious allergic reaction to eggs or to a vaccine
2. Have a fever, acute respiratory infections, or other infections or illness
3. Have a history of Guillin-Barre Syndrome
