2008 FLU VACCINE PROGRAM

STATUS: The 2008 Benefit Options Wellness Flu Vaccine Program will run October 1st through December 31st. The Wellness website will be updated throughout the 2008 Flu Vaccine Program to offer the most current information.

There are several options for eligible participants to receive FREE flu shots during the 2008 Vaccine Program.

Please note: all flu shot schedules are subject to change based on vaccine availability. No appointments needed – All events are first come, first serve.

If you are located in a rural area and there are no Statewide or Public events scheduled near you, you may call Healthwaves to find an alternative. Call Healthwaves at 480-968-1886 and please identify yourself as a State of Arizona employee/Benefit Options member. Healthwaves has agreements with some other organizations or may be able to accommodate reasonable requests.

WARNING
You should not receive vaccine if you:
1. Have ever had a serious allergic reaction to eggs or to a vaccine
2. Have a fever, acute respiratory infections, or other infections or illness
3. Have a history of Guillin-Barre Syndrome
