The Mayo Clinic Health Assessment is a confidential online questionnare on the EmbodyHealth portal www.bewellstaywell.az.gov The Health Assessment gathers information to help employees prioritize health risks and provides programs to motivate employees into adopting healthy lifestyle habits.
